Costco Optical accepts many vision and insurance plans for which our clinic is not credentialled to provide services. We are providers for:

• Vision Service Plan (VSP)

• United Health Care/Spectera

• Costco Employee Vision Plan

 

We verify insurance eligibility in accordance with the plans - please note that this requires personal information like:

• patient's name and date of birth

• plan sponsor's name and date of birth

• plan sponsor's last 4 of social security number.

 

If we are unable to verify benefits, we will provide a bill of services that you can submit to your plan for reimbursement. Call or email us if you have any questions.
